Is quality lost in extraction?

If the video's audio is already compressed and you pick a lossless target (WAV/FLAC), no transcoding happens. Otherwise, one re-encode is unavoidable.

Can I extract only part of the audio?

Not from this tool. Extract the full track first, then trim with the audio cutter or server-side trimmer.

Does the video leave my device?

Yes — this tool is server-side. The upload is deleted immediately after processing.
